INSTRUCTIONS TO THE COMMITTEE ON INVESTIGATION
KINDLY COMPLETE AND SUBMIT THIS TO THE SECRETARY IMMEDIATELY UPON ARRIVING AT YOUR DECISION. DO NOT RELY UPON BEING PRESENT TO MAKE A VERBAL REPORT, AS THE REPORT OF THIS COMMITTEE IS DESIRE SHORTLY AFTER THE LODGE IS OPENED AND THE SECRETARY IS EXPECTED TO HAVE ALL REPORTS IN HAND BEFORE THE OPENING OF THIS LODGE.
THE PETITIONER CANNOT BE BALLOTED UPON UNLESS THE ENTIRE COMMITTEE MAKE A COMPLETE REPORT.
IT IS ESSENTIAL THAT EACH MEMBER OF THE COMMITTEE INTERVIEW THE PETITIONER PERSONALLY
The following questions must be answered in full:

1. Does the Petitioner possess sufficient knowledge and intelligence to appreciate the value, the doctrines and tenets of Freemasonry?*
2. Is he financially able to afford the entrance fee and the payment of annual dues without hardship to himself or family?*
3. Is he in good health and not likely to become a burden upon the Craft?*
4. What are his physical infirmities, if any?*

6. Is his disposition such that he is likely to become a disorganizer and a creator of discord among the Craft?*
7. Is he a man with whom you and other Masons would willingly associate?*
8. Is he a peaceful and law-abiding citizen?*
9. Has he ever been engaged in plots against the Government of the Country or State?*
10. Has he ever been indicted, plead guilty or been convicted of a crime?*
11. Does he believe in the existence of Deity?*
12. Is his desire to become a member prompted by selfish or mercenary motives?*
13. Have you interviewed the Petitioner personally?*
14. Give any other comments you may deem essential.
